Your First Visit
We appreciate you choosing Ohio Vein. Your first visit with us is when we get to know you and your health. In an effort to make the most of our time at your first visit, we ask that you complete two forms in advance. Simply print out the proper Medical History Form – either “men” or “women” and the Venous Health History form. Fill them out as best you can and bring them with you. Thank you.
